🩺 Rudimentary Horn: A Rare Uterine Anomaly With Serious Implications

A rudimentary horn is an underdeveloped uterine structure that may be present in women with a unicornuate uterus.

It may be classified according to:

• Communication: Communicating or non-communicating with the main uterine cavity.

• Function: Functional or non-functional, depending on the presence of functional endometrial tissue.

🤔 Why Is a Rudimentary Horn Important?

A non-communicating functional rudimentary horn may cause menstrual blood to become trapped within the horn, leading to:

• Severe dysmenorrhea

• Chronic or recurrent pelvic pain

• Hematometra

• Endometriosis due to retrograde menstruation

However, the most serious complication is:

⚠️ Rudimentary Horn Pregnancy

Although rare, pregnancy can occur within a rudimentary horn, even when there is no direct communication with the main uterine cavity.

This may occur through transperitoneal migration of sperm or a fertilized ovum.

Because the rudimentary horn has limited capacity to accommodate a growing pregnancy, uterine rupture may occur, often during the second trimester.

This can result in life-threatening intra-abdominal hemorrhage.

Early diagnosis is therefore critical.

🔎 When Should We Suspect Rudimentary Horn Pregnancy?

Clinical suspicion should increase when ultrasound reveals:

• A gestational sac separate from the main uterine cavity

• An asymmetric uterine appearance

• No visible continuity between the pregnant horn and the cervix

• Myometrial tissue surrounding the gestational sac

3D ultrasound can help define the uterine anatomy, while MRI may be useful when the diagnosis remains uncertain.

🏥 How Is It Managed?

Rudimentary horn pregnancy requires prompt specialist assessment and surgical management.

Surgical excision of the pregnant horn, usually with the ipsilateral fallopian tube, is the standard definitive treatment because of the substantial risk of rupture and severe hemorrhage.

In selected patients, additional preoperative measures may be considered according to gestational age and clinical stability.

It may remain undiagnosed until a woman develops severe pelvic pain, menstrual complications, or a potentially life-threatening pregnancy.
