Happy Africa Day 2026

 ๐ŸŒAfrica is not only where I work…

it became part of who I am.

For 7 years in Zambia,

I have witnessed the strength of African women up close.

Their patience.

Their resilience.

Their love for their children despite every challenge.

As doctors, we treat patients.

But sometimes… Africa teaches us too.

It teaches humanity.

Community.

Strength during difficult days.

And how a mother can still smile while carrying so much silently.

On this Africa Day,

I celebrate the women who keep going no matter how hard life becomes.

The mothers.

The daughters.

The healthcare workers.

The women silently holding entire families together. ๐Ÿ’š

Happy Africa Day ๐ŸŒ

YOUR UTERUS IS SLIGHTLY HEART SHAPED

๐Ÿฉบ Arcuate Uterus: Is a Heart-Shaped Uterus Something to Worry About?

“Your uterus is slightly heart-shaped.”

Many women become worried when they hear this during an ultrasound examination.

But in many cases, the finding is simply:

๐Ÿฉบ An Arcuate Uterus.

YOUR UTERUS IS SLIGHTLY HEART SHAPED

๐ŸŒท What Is an Arcuate Uterus?

An arcuate uterus is characterized by a mild, shallow indentation at the top of the uterine cavity.

It is generally considered a normal anatomical variant, rather than a clinically significant congenital uterine anomaly.

  • Many women never know they have it.

  • Most have normal fertility.

  • Many experience completely healthy pregnancies.

๐Ÿ”Ž How Is It Discovered?

An arcuate uterus may be identified incidentally during:

  • Fertility investigations

  • Pregnancy ultrasound examinations

  • Recurrent miscarriage evaluation

  • Routine pelvic ultrasound

In most cases, its presence does not explain infertility or pregnancy complications.

⚠️ Arcuate Uterus vs. Septate Uterus

An arcuate uterus is NOT the same as a septate uterus.

An arcuate uterus has a shallow indentation of the endometrial cavity.

A septate uterus has a more pronounced internal partition that may be associated with adverse reproductive outcomes.

Distinguishing between these conditions is important because their clinical implications and management differ.

3D transvaginal ultrasound can provide detailed assessment of the uterine cavity and external fundal contour when the diagnosis is uncertain.

๐Ÿฉบ Does an Arcuate Uterus Need Treatment?

In most cases, no.

  • ๐ŸŒท No surgery is needed.

  • ๐ŸŒท No specific treatment is required.

  • ๐ŸŒท Normal fertility and pregnancy outcomes can be expected in most women.

  • ๐ŸŒท Reassurance is often the most important part of management.

An arcuate uterus alone is not an indication for hysteroscopic surgery.

❤️ The Key Message

Not every variation in uterine shape represents a dangerous diagnosis.

An arcuate uterus is usually a harmless anatomical variation.

Accurate diagnosis helps prevent unnecessary anxiety, investigations, and surgical interventions.

Sometimes, the best treatment is simply reassurance.

UTERINE ANOMALIES

UTERINE ANOMALIES

๐Ÿฉบ Mรผllerian Anomalies: Understanding Different Uterine Shapes

“Your uterus is abnormal.”

A sentence that can immediately create fear and anxiety.

But here is something important every woman should know:

Not every uterine anomaly is dangerous.

And not every unusual uterine shape causes infertility.

๐ŸŒท What Are Mรผllerian Anomalies?

Mรผllerian anomalies are congenital variations in the development of the uterus and, sometimes, the cervix and vagina.

They occur during fetal development, when the reproductive structures are forming.

Some variations are very mild and may never affect fertility or pregnancy.

Others may be associated with reproductive complications and require closer monitoring or specific treatment.

๐Ÿ”Ž How Are They Discovered?

Many women are unaware that they have a uterine anomaly until it is identified during:

  • Routine pelvic ultrasound

  • Fertility investigations

  • Pregnancy ultrasound examinations

  • Recurrent miscarriage evaluation

In some cases, the diagnosis is made incidentally, even when a woman has no symptoms.

๐Ÿฉบ What Will We Discuss in This Series?

In this educational series, we will explore different uterine shapes and congenital Mรผllerian anomalies, including:

  • Arcuate Uterus – A mild variation in uterine cavity shape, generally considered a normal anatomical variant.

  • Septate Uterus – A uterus containing an internal septum that partially or completely divides the cavity.

  • Bicornuate Uterus – A heart-shaped uterus with two uterine horns.

  • Unicornuate Uterus – A uterus that develops predominantly from one Mรผllerian duct.

  • Uterus Didelphys – A condition characterized by two separate uterine structures.

  • Rudimentary Horn – An underdeveloped uterine structure that may accompany a unicornuate uterus.

๐Ÿคฐ What Do These Conditions Mean for Fertility and Pregnancy?

Throughout this series, we will explain:

  • ✔️ How different uterine anomalies may affect fertility

  • ✔️ Their possible association with miscarriage

  • ✔️ Pregnancy risks and outcomes

  • ✔️ The role of ultrasound and other diagnostic techniques

  • ✔️ When treatment may be necessary

  • ✔️ When no intervention is required

Because an accurate diagnosis does not always mean that treatment is needed.

❤️ The Key Message

A different uterine shape does not automatically mean an unhealthy uterus or an unsuccessful pregnancy.

Every woman deserves an accurate diagnosis, clear information, and individualized care.

Understanding the diagnosis matters more than fearing its name.

๐Ÿฉบ Happy Labour Day: Celebrating Dedication, Responsibility & Rest

Happy Labour Day! ๐ŸŒท

Work means different things to different people.

For some, it is part of a daily routine.

For others, it is a responsibility that continues even on a day off.

Happy Labour Day

❤️ The Meaning of Work in Healthcare

As doctors, we learn that our profession is more than a job.

It is a commitment to caring for others, making difficult decisions, and being present when people need us most.

But we also learn something equally important:

Dedication does not mean we should forget to care for ourselves.

๐ŸŒท The Importance of Rest

Today is a reminder to appreciate both:

  • The moments we serve others.

  • The responsibilities we carry.

  • The effort we put into our work.

  • The moments of rest that allow us to recover and continue.

Rest is not a lack of dedication. It is an essential part of sustaining it.

๐Ÿค A Message of Appreciation

To every healthcare professional, every worker, and every person giving their best each day:

Your dedication matters. Your efforts deserve recognition. And your well-being matters too.

Whether you are working today or enjoying a well-deserved break, this day is for you.

Cord presentation ( Prolapse) Obstetrics Emergency

๐Ÿ’š Cord Prolapse: When Every Minute Matters in Obstetrics

“I didn’t rush for a C-section. I responded to a cord prolapse.”

One of the most dangerous obstetric emergencies can begin quietly.

And within minutes, everything can change.

A patient in early labour.

A twin pregnancy.

The first baby in breech presentation.

Suddenly, umbilical cord prolapse.

At that moment, it is no longer about waiting.

It is no longer about saying, “Let's observe.”

It becomes about one thing:

TIME.

Cord presentation ( Prolapse) Obstetrics Emergency

⚠️ What Is Umbilical Cord Prolapse?

Umbilical cord prolapse occurs when the umbilical cord descends through the cervix alongside or beyond the presenting part after rupture of the membranes.

The cord may become compressed between the baby and the maternal pelvis, reducing blood flow and oxygen delivery to the baby.

This can rapidly lead to fetal hypoxia and serious complications.

Every minute matters.

๐Ÿฉบ What Should Happen Immediately?

Cord prolapse is an obstetric emergency requiring immediate action.

The priorities are:

  • Relieve cord compression by manually elevating the presenting part when appropriate.

  • Optimize maternal positioning, such as knee-chest or left lateral positioning with pelvic elevation.

  • Call for immediate obstetric and neonatal assistance.

  • Prepare for urgent delivery, usually by emergency Caesarean section unless vaginal birth is imminent and can be achieved safely.

  • Arrange immediate transfer to an appropriate facility if definitive obstetric care is unavailable.

These measures aim to protect fetal oxygenation while definitive delivery is arranged.

⏱️ Why Does the Timing Matter?

Sometimes people say:

“Doctors are too quick to perform C-sections.”

But obstetrics teaches us something important:

Delay can be far more dangerous than a timely decision.

An emergency does not always begin with dramatic symptoms.

Sometimes, the warning signs are subtle.

And recognizing them early requires clinical judgment, experience, and the courage to act.

๐Ÿ’š When Referral Becomes a Life-Saving Decision

Sometimes, saving a life does not mean holding the scalpel.

Sometimes, it means:

  • Recognizing danger early

  • Making the right clinical decision

  • Initiating emergency measures

  • Communicating clearly with the receiving team

  • Ensuring the patient reaches definitive care without unnecessary delay

Timely referral is not “just a referral.”

It is an essential part of emergency obstetric management.

And sometimes, it makes the difference between tragedy and two healthy babies.

❤️ The Key Message

Cord prolapse does not wait. Neither should we.

Not every Caesarean section is performed because a doctor is impatient.

Sometimes, it is the intervention required to prevent irreversible harm.

And sometimes, the most important decision is recognizing when the patient needs urgent care beyond the available resources.

Because in obstetrics,

TIMING IS SURVIVAL.

She Wasn’t Cruel. She Was Scared

๐ŸคฐShe was not trying to be cruel

She was just… scared.

A recent case in Zambia described a woman attempting to end a late pregnancy alone, unsupported, and afraid.

The baby survived.

Against every attempt. Against every odd.

But this is not just a story about survival.

It’s a story about what happens when a woman feels she has no safe place to turn.

As an Obstetrician & Gynecologist, I want to highlight something important:

Unsafe attempts to terminate pregnancy  especially in advanced gestation are medically dangerous and unpredictable.

At this stage, the uterus is already prepared for labour.

Interventions outside a proper medical setting can lead to:

• Severe hemorrhage

• Uterine rupture

• Sepsis and life-threatening infections

• Incomplete delivery requiring emergency surgery

• Neonates born alive but in critical condition

And most importantly…

They put the mother’s life at real risk.

But behind the medical risks, there is always a story.

Women in these situations are often facing:

• Fear of abandonment or partner rejection

• Financial instability

• Social stigma

• Mental and emotional distress

And when there is no support system…

fear takes control.

This is why early access to care matters.

This is why safe, confidential medical counselling matters.

This is why we  as healthcare providers  must create a space where women feel safe to speak before reaching a crisis.

Because prevention is not just medical.

It is emotional, social, and human.

Sometimes…

saving a life starts by simply listening.

Personalized Medicine In Women Health

๐Ÿ’ฅ Personalized Medicine in Women’s Health: Because Every Woman Is Different

“Still receiving treatments that don't feel right for you?” ๐Ÿค”

Not every woman is the same.

So why should every woman receive exactly the same treatment?

Your hormones may be different.

Your genetics may be different.

Your medical history, reproductive goals, and stage of life are different.

And your healthcare should reflect those differences.

This is where Personalized Medicine in Women's Health comes in.

Personalized Medicine In Women Health

๐Ÿง  What Is Personalized Medicine?

Personalized medicine means tailoring healthcare decisions to a woman's individual needs, clinical characteristics, and personal circumstances.

Rather than relying on a one-size-fits-all approach, treatment decisions may consider:

  • Age and reproductive stage

  • Medical and family history

  • Hormonal and metabolic health

  • Genetic factors, when clinically relevant

  • Fertility and pregnancy goals

  • Previous response to treatment

  • Individual preferences and priorities

The goal is to provide the right care for the right patient at the right time.

๐ŸŒท Personalized Care in Fertility

Every fertility journey is different.

Two women with the same diagnosis may respond differently to treatment.

Personalized fertility care may involve:

  • Individualized ovarian stimulation protocols

  • Assessment of ovarian reserve when appropriate

  • Adjustment of medication doses according to ovarian response

  • Selection of fertility treatments based on age, diagnosis, and reproductive goals

Treatment should respond to the patient's needs, not simply follow a standard template.

๐Ÿคฐ Personalized Care During Pregnancy

Every pregnancy carries its own combination of risks and protective factors.

Individualized antenatal care may include:

  • Early assessment of maternal risk factors

  • Screening for preeclampsia and gestational diabetes

  • Low-dose aspirin prophylaxis when indicated

  • Ultrasound surveillance based on maternal and fetal risk

  • Individualized delivery planning

The aim is to identify potential complications early and provide appropriate care without unnecessary interventions.

๐Ÿฉบ Personalized Treatment in Gynecology

Women with similar symptoms do not always need identical treatment.

For example:

  • Fibroids: Management depends on symptoms, fibroid location, size, and fertility goals.

  • Endometriosis: Treatment considers pain severity, fertility plans, and previous treatment response.

  • Menstrual disorders: Investigation and treatment depend on the underlying cause.

  • Menopause: Hormonal and non-hormonal options are selected according to symptoms, medical history, risks, and preferences.

The same diagnosis does not always require the same treatment.

๐Ÿ’š Personalized Prevention and Wellness

Women's health extends beyond treating existing conditions.

Preventive care can also be individualized through:

  • Nutrition and lifestyle counseling

  • Appropriate physical activity recommendations

  • Metabolic risk assessment

  • Age-appropriate screening

  • Preconception and menopause counseling

However, personalization does not mean that every woman requires extensive hormonal or genetic testing.

Good personalized care is guided by clinical evidence, not unnecessary investigations.

❤️ Why Does Personalized Medicine Matter?

Because every woman deserves to be heard.

Her symptoms matter.

Her concerns matter.

Her medical history matters.

And her preferences should be part of the decision-making process.

Personalized care is not about giving more treatment.

It is about choosing the most appropriate treatment.

๐ŸŒท My Message to Every Woman

At Dr Rabab Cares, we believe that healthcare should recognize the individual behind every diagnosis.

Not every treatment works equally well for every woman.

And not every patient needs the same investigations, medications, or procedures.

Your treatment should be designed around you, not around a general template.

Because every woman is different.

And every woman deserves personalized care. ❤️

When Fibroid Location Matters More Than Size

✨ When Fibroid  Location Matters More Than Size

Not all fibroids behave the same.

And not all pregnancy losses are chromosomal.

This was a 12-week pregnancy.

The baby was well.

No obvious fetal abnormalities.

But there was a large fundal fibroid about 12–13 cm.

And the placenta was located directly beneath it.

Over time, the fibroid underwent extensive degeneration and necrosis.

One month later… the pregnancy ended in a missed miscarriage.

๐Ÿ’ฅWhat can we learn from this?

Most fibroids in pregnancy are harmless.

But when:

• The fibroid is large

• Located at the fundus

• Shares the same implantation site as the placenta

• And undergoes significant degeneration

The uteroplacental environment may be compromised.

Not always dramatically.

Sometimes gradually.

Sometimes silently.

Chronic vascular stress.

Mechanical distortion.

Inflammatory mediators from necrotic tissue.

All can affect placental perfusion.

Yesterday, we performed a myomectomy after about 8 weeks of that miscarriage.

Not because fibroids always cause miscarriage.

But because in this specific case, anatomy mattered.

And preparation for a safer future pregnancy matters even more.

Medicine teaches us humility.

Sometimes it’s not about doing more.

It’s about understanding better.

Medicine Is Not Just Diagnosis and Protocols

Medicine is not just about diagnoses and protocols

In obstetrics, every decision carries a story, a family, and a future.

Some days are joyful, some are heavy, and many are quietly exhausting.

Behind every ultrasound, every delivery, and every long night on call,

there is a responsibility we carry with humility and care.

I’m grateful for the trust women place in us during the most vulnerable moments of their lives.

This work reminds me daily that being a doctor is not about perfection

it’s about presence, honesty, and doing your best, even when outcomes are beyond your control.

When Trust Is Built Over Time๐Ÿค—

In obstetrics, trust is not built in a single visit.

It is built over time '

through listening more than speaking,

through honesty during difficult conversations,

and through being present when it truly matters.

Continuity of care is often underestimated.

Yet it plays a crucial role in maternal outcomes, emotional safety, and informed decision-making.

When a woman chooses the same doctor again, it reflects more than medical care

it reflects reassurance, understanding, and peace of mind.

As obstetricians, this trust is both an honor and a responsibility.

A responsibility to remain consistent, evidence-based, compassionate,

and fully aware that every pregnancy carries its own story, fears, and hopes.

Grateful for the quiet moments that remind us why we chose this path.

Grateful for trust that is earned, not asked for.

Happy New Year 2026

2026 is not about noise or speed.

It’s about clarity, intention, and growth.

Not every quiet path is empty.

Some are full of meaning, purpose, and wisdom.

This year, I am choosing: Peace over pressure,

Light over constant visibility,

Patience over rushing outcomes,

Health over burnout,

Family over endless obligations,

Depth over numbers.

And a calm, grounded mind over chaos.

I’m learning that growth doesn’t always announce itself.

Sometimes, it happens quietly

in boundaries, in consistency,

in choosing what truly matters.

Trusting the process,

placing my steps in God’s hands,

and moving forward with faith and gratitude.

Quietly becoming myself.

Thrombocytopenia In Pregnancy (Real story)

 ✨ Behind the story…

Yesterday, we talked about thrombocytopenia in pregnancy —

numbers, guidelines, thresholds, and decisions.

Today, I want to talk about what those numbers really mean in real life.

Because behind every “routine” cesarean section,

there can be a quiet story no one sees.

A mother whose platelets were slowly dropping.

Weeks of careful monitoring.

Daily questions in the doctor’s mind:

Observe… or intervene? Wait… or act?

No panic.

No dramatic alarms.

Just vigilance.

Because postpartum bleeding doesn’t always come crashing through the door.

Sometimes, it whispers.

A uterus that tightens… then relaxes.

Blood that collects silently.

Signs that only show themselves if you’re truly watching.

In those moments, medicine is not about heroics.

It’s about being prepared before the crisis exists.

✔ Blood ready

✔ Platelets available

✔ Team aligned

✔ Decisions made calmly, minute by minute

This was not a dramatic emergency.

And that’s exactly why it mattered.

Because the most dangerous situations are often the quiet ones.

The ones that tempt us to relax too early.

The ones that remind us that stability is not a guarantee — it’s something we protect.

Thankfully, this story ended well.

A stable mother.

A safe outcome.

A reminder of why preparation saves lives.

Every birth deserves respect.

Every mother deserves safety.

And every “routine” case deserves our full attention.

Doctors Are Human Too

 ๐Ÿ’› Doctors Are Human Too

Being a doctor doesn’t mean being available 24 hours a day.

We get tired.

We need rest.

We have families, homes, sleep, and a life outside the hospital.

Sharing my number with a patient is an act of trust

not an invitation to call at midnight for non-urgent issues,

and not a permission to cross personal boundaries.

If someone has been unwell for hours, it’s unfair to wait until 12 AM to call the doctor  and then refuse to visit the hospital.

What meaningful medical help can be given over the phone at that time?

Respect goes both ways.

Every physician deserves:

• Clear working hours

• Resting time

• Privacy

• And the basic courtesy of being contacted at reasonable hours  unless it's a true emergency

Being reachable does not mean being on call for the whole world.

Let’s remember: doctors are humans, not machines.

Respecting a doctor’s boundaries is part of respecting their care.
