๐ PMOS Phenotypes & Different Presentations
Not every woman with PMOS looks the same.
PMOS can present in different ways depending on which features are present:
- Ovulatory dysfunction
- Hyperandrogenism
- Polycystic ovarian morphology (PCOM)
Based on these features, PMOS can be classified into four main phenotypes.
๐ฅ Phenotype A — Classic Phenotype
Hyperandrogenism + Ovulatory Dysfunction + Polycystic Ovarian Morphology
All three features are present.
๐ฅ Phenotype B — Classic Non-PCOM Phenotype
Hyperandrogenism + Ovulatory Dysfunction
Polycystic ovarian morphology is not required for this phenotype.
๐ฅ Phenotype C — Ovulatory Phenotype
Hyperandrogenism + Polycystic Ovarian Morphology
Ovulation is generally preserved.
๐ฅ Phenotype D — Non-Hyperandrogenic Phenotype
Ovulatory Dysfunction + Polycystic Ovarian Morphology
Without clinical or biochemical hyperandrogenism.
๐ค Why Do Women With PMOS Have Different Symptoms?
This explains why two women with PMOS may have completely different presentations.
- One may mainly experience irregular menstrual periods.
- Another may struggle with acne or excessive hair growth.
- Another may experience fertility challenges.
- And another may have metabolic abnormalities, even without obvious symptoms.
The combination and severity of symptoms can vary considerably from one woman to another.
๐ค The Important Message
PMOS is not one single picture.
It represents a spectrum of reproductive, hormonal, and metabolic features.
Management should therefore be individualized according to:
- Symptoms and clinical presentation
- Metabolic risk
- Menstrual and ovulatory function
- Fertility goals
- Overall health
Different presentations require individualized care.
