๐ธ PMOS Explained
Episode 7
๐ท PMOS Through Different Life Stages
๐ก PMOS does not look exactly the same throughout a woman’s life.
Its features, priorities, and health risks can change with age.
๐ง During Adolescence
Irregular menstrual cycles can be part of normal puberty, especially during the first year after menarche.
๐ That is why diagnosing PMOS in teenagers requires extra care.
In adolescents, both of the following are required for diagnosis:
• Ovulatory dysfunction
• Hyperandrogenism
⚠️ Ultrasound and AMH are not recommended for diagnosing PMOS in adolescents because normal pubertal development can mimic its features.
๐ธ During the Reproductive Years
PMOS may present with:
• Irregular or absent periods
• Acne or excess hair growth
• Difficulty with ovulation
• Fertility concerns
• Weight or metabolic problems
๐ Management depends on the woman’s symptoms, metabolic health, and whether pregnancy is currently desired.
๐คฐ During Pregnancy
Women with PMOS may have a higher risk of certain pregnancy complications, including:
• Gestational diabetes
• Hypertensive disorders of pregnancy
๐ฉบ Preconception health, blood pressure, weight, glucose status, lifestyle, and other medical conditions should therefore be optimized before pregnancy whenever possible.
๐ท During Perimenopause
Menstrual cycles naturally become more irregular during this stage, making previous PMOS symptoms harder to interpret.
Some androgen-related symptoms may persist, while long-term metabolic and cardiovascular health become increasingly important.
๐ The Important Message
PMOS is a lifelong condition, but its expression changes over time.
๐ Care should therefore evolve with the woman — from adolescence, through the reproductive years and pregnancy, and into later life.
The goal is not only to manage current symptoms but also to protect long-term health and quality of life.
